For Bethesda it's the initial map design along with setdressing, placement and some manual addition later. Also manual masking for specific terrain ID also involved. NMS actually made a bad impression for procedurally generated algorithm to many gamers... Also, I blame media and random twitter sheeps for these kindda negative outlook in a game. Well, people be more armchair-developers themselves than actually trying to understand what's happening under the hood and expand their knowledge.
