Spoiler After he got shot, he is most probably stuck in a limbo, think of it as a divine intervention, where the goddess Tawaret (lady hippo) - the goddess of fertility, will help the identities come to a solution or something in the next episode. Tawaret is an ally of Khonshu, so she is using her powers to stop Marc/Steve from dying. The overall look and location of a mental asylum was taken from a comic series where Marc was diagnosed with delusion of being a vigilante. Hope this clears up some of the things for you. They really captured the vibe of comics where even the reader is confused what's going on like the protagonist. I'm a sucker for Egyptian mythology and pyramids, the exploration bits were probably the best since The Mummy(1999).
